Documentation Available to Authorized Parties

Documentation may be available to authorized individuals or entities as permitted by applicable law, court order, signed releases, and our service agreements. All records are maintained in accordance with our confidentiality policies and applicable legal and ethical standards.

Documentation may include:

• Intake and enrollment documentation

• Attendance and scheduling records

• Factual visitation or exchange summaries

• Incident reports, when applicable

• Cancellations, no-shows, or policy violations

• Communication logs related to scheduling or service coordination

Reports are objective and limited to factual observations. Documentation does not include opinions, recommendations regarding custody or visitation, psychological evaluations, legal conclusions, or therapeutic assessments unless specifically contracted for and within the provider's scope of practice.

Documentation may be released only to authorized parties, which may include:

• The court, when ordered

• Attorneys of record, when appropriately authorized

• Guardians ad Litem (GALs)

• Child welfare agencies (e.g., DSS/CPS), when authorized or legally required

• Parents or legal guardians, as permitted by court order or signed authorization

• Other professionals authorized by a valid Release of Information or as otherwise required by law
